This resource, from The Office of Disability Services at Penn State University, provides grade-by-grade suggestions for preparing a student with a disability for post-secondary education.

This checklist contains tasks for ninth through twelfth grade, as well as after graduation. It also contains a list of internet resources for career exploration and college planning.
