Wearing a Medical Alert Bracelet Social Story

A medical alert bracelet has my information on it. It is about who I am and my health.

This might include my name and address. It also might list my emergency contact’s phone number.

It will list my health conditions, medicines, and allergies. It will also list anything else that doctors might need to know in an emergency.

These bracelets help doctors. They can learn about my health and make decisions.
